Community Service Hours Request

Please complete this application form if you are seeking community service hours for your school program(s), sport(s) team, scholarship or club/organization.

Once your form has been submitted, the Volunteer Coordinator will contact you via email from volunteer@lvnhm.org or volunteerlvnhm@gmail.com after your application has been reviewed. (Check your SPAM folders.)

Please, DO NOT arrive to volunteer at the museum before being contacted by the volunteer coordinator.

Once you have scheduled a volunteer shift with the Coordinator, please remember to bring any paperwork in for the site supervisor to sign.

We do not keep copies of forms on site.

Please be sure to provide your email and/or phone number in order to receive volunteer shift updates or adjustments to volunteer schedules. Communicate with the Volunteer Coordinator if your schedule will prevent you from completing your shift.

We cannot accommodate requests for hours less than 48 hours in advance, meaning, if you need hours completed within two days, we may not be able to help. You must request service hours with advance notice.

We cannot accommodate court-mandated hours at this time.
